Fee For Service
This model allows physicians to bill for each patient they have seen. The Comprehensive Care Model and Family Health Group model physicians are compensated primarily through FFS but are also eligible for specific bonuses and premiums based on patient enrolment.
Comprehensive Care Model (CCM)
Is for solo physicians who commit to provide comprehensive primary health care and a block of after-hours services each week to their enrolled patients.
Family Health Group (FHG)
Is offered to groups of three or more physicians to provide comprehensive primary health care and after-hours services to their enrolled/assigned patients.
Family Health Organization Model
The Family Health Organization (FHO)
Is an alternative payment model in Ontario’s Primary Care Renewal program, established to target grant new money to family physicians practicing comprehensive and preventive care. The FHO works on a “blended payment” model that provides family physicians increased income, steady monthly cash flow, funded continuing medical education as well as allowing for flexibility in the delivery of medical services.
Who can form or join a FHO?
A group of three or more family physicians is needed to form a FHO. The physicians do not necessarily have to practice in the same geographic area but they work together as a group to provide comprehensive primary care, extended office hours, and on‐call coverage to the group’s enrolled patients. Doctor’s may also join an existing FHO.

Physicians have more choice in how they deliver medical services, such as through Telehealth or referring patients to other health practitioners such as a specialist, nurse or dietician without penalty.

FHT: Family Health Team
Family Health Network (FHT) and Family Health Organization (FHO) models, which have six or more physicians, are compensated primarily through capitation payments. Physicians are also eligible for specific bonuses and premiums based on patient enrolment and preventative screening criteria. The key differences between the FHO compared to the FHT include the base rate payment, associated basket of core services, and access bonus calculation. FHT’s also work with other allied health professionals like Nurses, Social Workers and Pharmacist.
